Case Manager – FT – Hunt, TX
General Qualifications:
This position assumes primary responsibility for case management of assigned caseload. Duties include but are not limited to assessment, treatment planning, individual and group counseling, charting, discharge, and continuing care planning. The applicant must be licensed as an LCDC or other Qualified Credentialed Counselor. Experience working with chemically dependent patients is required. A bachelor’s or a graduate degree in a Human Services field is preferred. Basic computer skills are required. |
Patient Driver – FT – Hunt, TX
General Qualifications:
The Patient Driver assures the safe transportation of patients/visitors to and from La Hacienda. The position requires a valid Texas driver’s license, excellent driving record, pleasant and courteous demeanor and the ability to react quickly and safely in a time of crisis. Candidates must be at least 25 years old and flexible in the workday. |
